With reference to a meta-analysis this comment emphasizes the benefit of low-frequency ESWL. Unfortunately the author states that the longer duration of the treatment needs attention. But Anglada-Curado and co-workers addressed this problem and showed that with a lower shockwave frequency and better disintegration the total number of shockwaves can be reduced. Fine-tuning of the treatment strategy might hopefully show that with a better disintegration it might be possible to further reduce the total number of shockwaves. It is my personal assumption that most patients today are over-treated in terms of number of shockwaves at each ESWL session.
