This is a case report on a series of five ESWL sessions successfully carried out in a patient with uncorrected coagulopathy and thrombocytopenia. Fortunately this procedure was uneventful for the patient despite the situation, but to proceed with ESWL under these conditions cannot be recommended. The intention to focus on the stone and avoiding trauma to surrounding tissues is easy to say but difficult to do. Why general anesthesia was impossible in this case is not further explained, neither is the strategy if the patient had got a life-threatening bleeding.
