This letter to the Editor describes a case of subcapsular renal hematoma as a complication to SWL. This complication, although rare, is well recognized. The diagnosis always should be suspected in patients who present with unexpected pain after SWL.
Risk factors are hypertension and treatment with anticoagulation/antiplatelet agents. Conservative treatment is sufficient in most cases although the bleeding in this patient was managed with arterial embolization.
