Abstract

Technological advances in minimally invasive treatment of stone disease and its integration with concomitant clinical practice are amongst the most important achievements in urology. Despite the wealth of information accumulated over the years and the richness of existing literature, the knowledge about the definition, treatment and outcomes of residual stone fragments after percutaneous nephrolithotomy (PNL) is still insufficient. Due to the high stone load a lot of patients with staghorn stones have residual fragments (RFs) after treatment with PNL, which depends on the size of tract, definition of stone free rate (SFR), timing of evaluation and the imaging used. No consensus exists on the imaging modality or their timing in the evaluation of possible RFs. The treatment of residual stones is apparently different depending on the facilities of the department and the preference of the surgeon, which includes active surveillance, shock wave lithotripsy (SWL), retrograde intrarenal surgery (RIRS) or a second look PNL.

Salvi M. et al., 2020: Active treatment of renal stones in Pelvic Ectopic Kidney: systematic review of literature

Abstract

Purpose: To identify the role of Doppler US (DUS) in the prediction of factors affecting stone-clearance post-SWL for ureteral stones in addition to its role in prediction of pretreatment stone impaction.

Methods: We prospectively included patients with ureteric stone for SWL between October 2018 and September 2019. Patient's demographics were collected. Using DUS, resistive index (RI), Δ RI, and ureteric jets were evaluated in both kidneys. Also, stone site, volume, density, skin to stone distance, degree of hydronephrosis, ureteral wall thickness (UWT) were evaluated by NCCT. Univariate and multivariate logistic regression analyses were performed to identify factors that contribute to treatment success. Correlation between ipsilateral RI with other radiological findings predicting stone impaction was done.

Results: A total of 93 patients were finally included (61 males and 32 females). The mean age was 31.4 ± 7.7 years. The success rate (absence of significant residuals post 1 month by NCCT) was 51.6%. In multivariate analysis, the presence of ureteric jet, ipsilateral lower RI, and ipsilateral lower UWT were independent factors for treatment success (P = 0.0001, 0.002, and 0.03, respectively). Also, ipsilateral absence of ureteric jet, increased hydronephrosis, and increased UWT were correlated with higher ipsilateral RI in prediction of stone impaction. Ipsilateral higher RI was found to achieve higher accuracy in stone impaction prediction (PPV of 94.8%).

Conclusion: Pretreatment DUS is a useful, non-invasive with a less irradiative tool for prediction of stone impaction and stone-free rate post-SWL for ureteric stones.

The Transcranial Pulse Stimulation (TPS®) device, which was developed by STORZ MEDICAL in Switzerland, is the only medical device marketed in Europe that is approved in the European Union for the treatment of the central nervous system of patients with Alzheimer's disease. The effect of TPS® is now being investigated in another neurological condition: In September 2020, the world's first clinical study for the treatment of Parkinson's patients with shock wave therapy using real-time documentation started at MedUni Vienna (Department of Neurology) in Austria under the direction of Professor Roland Beisteiner.

A further milestone was reached on Monday. The ground-breaking ceremony and thus the start of construction of the new company building of STORZ MEDICAL Deutschland GmbH took place on 3rd August. To shorten the waiting time a little, we report in this issue about two professional sports teams from the hosting country. Read on for an insight into the world of professional sports in Japan as seen through the eyes of three trainers: two from basketball team ALVARK TOKYO and one from football team Júbilo IWATA. All of them are licensed judo therapists, a qualification based on a special Japanese osteopathy tradition known as »hone-tsugi« (»bone-setter«).

NEUROLITH installation: in the treatment of patients with Alzheimer’s disease, the »Rudolfinerhaus Privatklinik« in Vienna, Austria, relies on Transcranial Pulse Stimulation (TPS)

Throughout the world, more and more specialised medical centres are using Transcranial Pulse Stimulation (TPS®) with the NEUROLITH® system for the treatment of patients with Alzheimer’s disease. In June 2020, another NEUROLITH® system was installed successfully at the private hospital »Rudolfinerhaus Privatklinik« in Vienna, Austria, where it will be used under the medical direction of Dr Matthias Geisler. On-site training of the hospital’s treatment team was carried out by Dr Henning Lohse-Busch (Rheintalklinik, Bad Krozingen, Germany) who also played a decisive role in the development of TPS®.

Alzheimer’s treatment: study on STORZ MEDICAL’s Transcranial Pulse Stimulation (TPS) is among the top 10% most downloaded papers in Advanced Science

In December 2019, the first study1 on Transcranial Pulse Stimulation (TPS®) with STORZ MEDICAL’s NEUROLITH® system was published in the prestigious journal Advanced Science. In the study, Beisteiner et al. evaluate the TPS® method and present treatment results. The presented data shows that it is possible to improve brain performance with TPS® and that there is a long-term effect.

STORZ MEDICAL has received the CE approval for the MODULITH® SLC. The areas of application of this innovative shock wave system are ischaemic heart failure, coronary heart disease, and angina pectoris.

Being designed for penetration depths of between 0 and 150 mm, the MODULITH® SLC is well prepared to master any challenge in daily clinical practice, reaching target areas from the apex to the posterior wall of the myocardium. The proven shock wave technology developed by STORZ MEDICAL makes use of a cylindrical coil in combination with a parabolic reflector for focusing. The coil and reflector are perfectly adapted to the specific medical requirements. The long-life cylindrical coil lasts for several hundred treatments.
